Library Instruction
Request Order Form

 CSC faculty on the St. Paul campus may use this form to request a Library instruction session for their class. You will be notified by phone or e-mail to confirm that your class is scheduled, or to discuss further details. Please submit one form for each instructional session. 

Note to TRW instructors: Librarians are assigned to instruct TRW classes as our schedules allow; we will notify you as to who will teach your class. Please allow one hour for Library instruction. 

Note to instructors in other departments: You may request that a certain librarian instruct your class, and this request will be forwarded to that person. If you are not sure which librarian is assigned to your department, please consult our listing of Librarian Liaisons.
